The
City Of Mountlake Terrace is an acceptance agent for the United
States Passport Services. A U.S.
Passport is an extremely important document. Extra precautions
are taken to ensure the integrity of the passport application
process.
The City also provides
passport photo services at a cost of $14.00 per person.
Passport
Requirements
(ALL of the following six items are required)
- Certified
Birth Certificate or previously issued US Passport or Certificate
of Naturalization. Children 15 and under who have previously
been issued a passport must submit the passport and a certified
long form copy of their birth certificate. Hospital certificates
of birth are NOT acceptable.
- Two
identical current passport photos – 2x2 square, with
a white or off-white background. Photographs must meet U.S.
Passport standards. You may either bring acceptable photos
or you may purchase photos here for $14.00 when you apply
for your passport.
- Valid
Drivers License or State issued ID card
- Social
Security Number for all applicants regardless of age
- Check
or Money Order only to PASSPORT SERVICES
Adults (16 and older) $110.00 each – passport valid
10 years
Minors (15 and younger) $80.00 each – passport valid 5 years
Expedited Adults $170.00 each
Expedited Minors $140.00 each
- Separate
Check, Money Order or Cash to CITY OF MOUNTLAKE TERRACE
for service fee of $25.00 per application and, if appropriate,
photo fee of $14.00 per applicant.
All
Applicants Must Appear in Person (Adults and Minors)
Children
under age 16: Both parents need to be present with
child (if not possible please call 425-776-1161 for more information).
Children, under 15, who previously had a passport must present
their previous passport and a certified copy of their long
form birth certificate.
Children
16-17: one parent needs to be present with child.
Passports
are mailed directly to you and generally take 6 to 8 weeks.
Expedited Service makes it possible to receive your
passport in about 2 to 3 weeks and is available for an additional
$60.00 per application.
